Job summary

Homeward Pikes Peak in Colorado Springs is seeking a Lead Case Manager to support The Commons Permanent Supportive Housing. You will lead a team of case managers, carry a caseload, and coordinate services to promote housing stability for Veterans, families, and individuals.

The role requires 3–5 years in human services, a Bachelor’s in Social Services, HIPAA knowledge, and strong documentation skills. It is a full-time exempt position with comprehensive benefits.

Qualifications

• Bachelor’s degree in Social Services required.
• 3–5 years of direct client supportive services experience.
• First Aid/ CPR certification or willing to obtain within 3 months.
• Experience working with diverse populations and homelessness-related challenges.
• Knowledge of privacy/confidentiality standards in counseling and billing.

Responsibilities

• Lead case management for The Commons PSH community and carry a caseload.
• Coordinate with staff, vendors, and housing partners to support housing stability.
• Maintain accurate client records and HMIS documentation.
• Develop individualized service plans and monitor progress toward goals.
• Assist with staff training and policy implementation.
• Provide transportation as needed and collaborate with community agencies.
• Ensure compliance with HIPAA and organizational privacy standards.
