Job summary

Qualifications

• Associate’s degree is required; may substitute with five years of direct case management experience with homeless individuals.
• Knowledge of HIPAA and privacy standards; crisis management and harm reduction principles.
• Working knowledge of Internet and Microsoft Office products including Word, Excel, and Outlook.
• Capable of partnering with and empowering program residents.
• Cultural sensitivity toward diverse populations.

Responsibilities

• Establish and maintain professional, supportive relationships with program participants.
• Provide individualized case management using Housing First, trauma-informed, person-centered, and strengths-based approaches.
• Complete initial and ongoing assessments to identify participant strengths, needs, barriers, risks, and goals.
• Develop and update individualized service plans based on participant priorities.
• Review service plans as changes occur and conduct home visits as required.
• Maintain HMIS data and participate in CoC meetings.
• Assist participants with housing transitions and stability strategies.
