Job summary

The Salvation Army in Knoxville, TN is seeking a dedicated Case Manager to lead direct service delivery within the VA GPD Program. You will interview clients, accept referrals, and provide comprehensive, long-term case management for an assigned caseload, tailoring program plans to each client’s history and needs while ensuring compliance with funding requirements.

You will supervise and mentor a team of case management staff, develop short- and long-term goals, advocate for access to housing

Qualifications

• Bachelor’s degree in Social Work, Behavioral Science, or related field is required.
• 3–4 years of progressively responsible direct case management experience is required.

Responsibilities

• Lead worker to case management staff, interview, accept, and provide comprehensive, long-term case management services for assigned clients in the VA GPD Program.
• Develop comprehensive program plans/goals and evaluate client progress through mentoring and counseling sessions.
• Advocate for clients to obtain services to help them function in their environment.
• Ensure compliance with funding requirements and maintain HMIS records.
