Job summary

PATH (People Assisting The Homeless) is seeking a Lead Case Manager for the Hollywood Inn Express South site in Los Angeles to guide clients from street to housing. You will manage a direct caseload, coordinate with a multidisciplinary team, and support program operations under the supervision of the program manager.

The role requires experience in healthcare or homeless services, strong communication and organizational skills, and a commitment to equity.

Responsibilities

• In collaboration with QA, ensure programs comply with contracts and partnerships.
• Serve as a contact for program matters under the program manager.
• Provide administrative oversight of the team and cover for the program manager when needed.
• Assist with critical incident reports per PATH policies and contracts.
• Conduct intake and needs assessments; develop ISP addressing housing goals and services.
• Provide referrals and connections to resources supporting ISP goals.
• Complete program-specific assessments (e.g., VI-SPDAT).
• Maintain thorough documentation and client files with current records.
• Build therapeutic relationships with clients and partners to support housing outcomes.
• Provide crisis intervention and risk assessment to enhance safety and independence.
• Use motivational interviewing and trauma-informed care in service delivery.
• Maintain knowledge of community resources for wrap-around services.
