Job summary

PATH, People Assisting The Homeless, is seeking a Case Manager for the Hotel Silver Lake Interim Housing team in Los Angeles. You will support clients through housing pathways, coordinate services, and help develop individualized plans to address health, income, and housing goals.

Responsibilities

• Conduct intake and assess participants to develop ISP.
• Provide referrals and support to achieve ISP goals.
• Complete program assessments (e.g., VI-SPDAT).
• Evaluate progress and conduct 90-day reviews with participants.
• Maintain accurate participant files and confidentiality.
• Provide crisis intervention and trauma-informed support.
• Build relationships with clients and partners to support housing goals.
