Job summary

PATH is seeking a Case Manager for our Alma Apartments site in Los Angeles, CA. This full-time, non-exempt role requires driving and will operate on-site in a dynamic supportive housing program.

You will conduct intake, assess needs, develop ISP, provide referrals, and support housing goals while maintaining HIPAA privacy and accurate documentation. This role emphasizes client-centered care and coordination with community resources.

Qualifications

• Experience in healthcare, case management, homeless services or similar field.
• All levels of education and experience are welcome.

Responsibilities

• Conduct intake and individualized needs assessment and develop ISP addressing housing goals and other client needs.
• Provide referrals, linkages, information, and support to resources to achieve ISP goals.
• Complete program-specific assessments (e.g. VI-SPDAT).
• Evaluate progress with 90-day reviews and maintain documentation per PATH policies.
• Maintain confidential client files and HIPAA compliance.
