Job summary

Primary Health Care, Inc (PHC) in Des Moines, IA, is hiring an Economic Development Case Manager to help clients experiencing homelessness gain employment, housing, and essential documents.

We offer four weeks of PTO in the first year, paid holidays, and PHC training opportunities. Responsibilities include intake, coordination with PHC services, and assisting with transportation and outreach.

Qualifications

• Required: Bachelor’s degree in psychology, counseling, mental health, human services or related field or equivalent.
• Effective written and verbal communication, team oriented, proficient in Microsoft Office.
• Excellent interpersonal skills with ability to maintain relationships and show empathy.
• Detail oriented with high accuracy; strong organization and prioritization.
• Licenses/Certifications: Mandatory Reporter Training and Chauffeur’s license, within introductory period.

Responsibilities

• Completes intakes and ongoing assessment with clients to identify needs and develop goals.
• Monitors and documents client contacts, needs, goals, progress and important events.
• Coordinates with PHC services and referrals to external agencies.
• Advocates for clients with community agencies, schools and other entities.
• Ensures transportation access to appointments per PHC policy.
• Communicates with agencies with proper releases of information; maintains discretion.
• Performs outreach at community venues and serves as liaison with PHC and community resources.
