Primary Health Care (PHC) in Des Moines, IA is seeking a Case Manager Team Lead for Homeless Support Services to lead assigned case management staff and advocate for clients within grant guidelines. The role involves coordinating services, conducting intakes, tracking outcomes, and collaborating with PHC teams and external agencies to connect clients with housing, healthcare, and support programs.

A Bachelor’s degree and 2 years’ experience are required.

• Bachelor’s degree or equivalent combination of education and experience.
• Minimum 2 years of experience in human services.
• Supervisory experience with ability to direct and provide feedback.
• Excellent verbal and written communication and listening skills.
• Strong organization and time management with ability to work independently.
• Leadership with ability to gather a team toward goals.
• Proficiency using Microsoft Office and internet-based applications; willingness to learn new software.
• Strong interpersonal skills and ability to de-escalate difficult situations.
• Detail oriented with high accuracy.
• Professionalism with empathetic interaction.

Aufgaben

• Coordinates case management duties for assigned area(s), ensuring key tasks are completed.
• Leads team meetings for the grant team and attends all grant related activities.
• Provides initial consultation for escalated client situations.
• Completes intakes and ongoing assessment with clients to identify needs, develop goals, and access resources and services.
• Monitors and documents client contacts, needs, goals, progress, and significant occurrences.
• Enters data into applicable data systems accurately and timely, maintaining productivity standards.
• Reviews entered data each month to ensure accuracy and timeliness.
• Collaborates with Program Director and Grant Accountant monthly to ensure grant spending is up to date.
• Facilitates coordination with other PHC services and referrals with external agencies.
• Advocates for clients with community agencies, schools, significant others and other appropriate agencies.
