Primary Health Care, Inc. seeks an Economic Development Case Manager to assist clients experiencing homelessness in Polk County, Des Moines, IA. The role supports housing and income goals, including resume help, document gathering, and transportation coordination.

The position is time-limited, grant-funded, with a strong emphasis on connecting people to community resources and PHC services, while offering professional development opportunities and a competitive benefits package.

Qualifications

• Bachelor’s degree in psychology, counseling, mental health, human services or other related field or equivalent combination of education experience.
• Effective written and verbal communication skills.
• Team oriented.
• Proficiency using Microsoft Office applications including Excel and other database applications.
• Excellent interpersonal skills with ability to establishes and maintain effective working relationships with a diverse group of individuals.
• Detail oriented with high degree of accuracy.
• Demonstrates professionalism with ability to maintain composure in all situations and show empathy.
• Organization and prioritization skills.

Responsibilities

• Completes intakes and ongoing assessment with clients to identify needs, develop goals, and access resources and services.
• Monitors and documents client contacts, needs, goals, progress, and significant occurrences, according to grant guidelines and PHC policies and procedures.
• Facilitates coordination with other PHC services, including medical, dental, mental health, and substance use, as well as referrals for services with external agencies, including emergency and crisis services.
• Advocates for clients with community agencies, schools, significant others and other appropriate agencies.
• Ensures clients have access to transportation to necessary appointments as appropriate, assisting with client transportation in accordance with PHC transportation policy.
• Communicates with other agencies on behalf of clients using professional discretion with client information, ensuring that appropriate releases of information are obtained.
• Conducts outreach activities at community agencies, community gatherings, neighborhoods, shelters, and other appropriate venues as requested. Serves as a liaison between PHC and community resources.
