Job summary

Optima Medical Management Group is seeking a Lead Care Manager (LCM) to develop personalized care plans, coordinate member services, and work with families and Primary Care Providers to ensure timely medical care.

The role focuses on a diverse California ECM population and requires a high school diploma, 1 year in case management or related care work, and a valid CA driver’s license. Spanish bilingual is required. The position is temporarily hybrid with travel to Stockton and San Ramon.

Qualifications

• High school diploma or GED required.
• 1+ year in case management, member care, or customer service.
• Valid California driver’s license and vehicle insurance required.
• MA certificate or medical terminology knowledge preferred.

Responsibilities

• Interview members to assess medical and social needs and develop individualized care plans.
• Respond to inquiries while ensuring compliance with hospital and legal requirements.
• Coordinate social services with interdisciplinary teams and arrange resources.
• Maintain ongoing contact with members via telehealth and in-person visits.
• Advocate for members, arrange appointments, and monitor progress.
• Provide empathy and professionalism in member communications.
• Support behavioral health coordination and community resources.
• Perform additional duties as assigned.
