Fresno Mission is seeking a Lead Care Manager to provide family-centered, member-focused care coordination for individuals eligible for Enhanced Care Management services in a California setting.

The Lead Care Manager will assess patient needs, coordinate resources, and advocate within managed care guidelines to improve health outcomes for high-need Medi-Cal beneficiaries.

Qualifications

  • AA or higher in Drug and Alcohol, Social Work, Human Services or related field.
  • Must be a mature Christian with ability to articulate Biblical beliefs and their application within the Fresno Mission context.
  • All employees are Christian missionaries and may participate in chapel services, Bible studies and prayer times.
  • HIPAA, confidentiality, PHI and release of information compliance required.

Responsibilities

  • Coordinate with the member’s service providers and support network to ensure a seamless experience and avoid duplication of services.
  • Actively engage ECM-eligible members.
  • Oversee ECM services and implementation of the care plan.
  • Advocate for ECM members with health care professionals.
  • Coordinate with hospital staff on ECM member discharge plans.
  • Monitor treatment adherence and provide health promotion and self-management training.
  • Carry a full caseload of 50 ECM members within policy guidelines.

Description The Lead Care Manager position at Fresno Mission provides family-centered, member-focused, relationship-based care coordination services to individuals eligible for and enrolled in Enhanced Care Management services. This position effectively utilizes Agency policies and procedures and makes appropriate independent judgments in the course of daily work, especially in consideration of Fresno Mission population of focus. The overall goal of CalAIM is to provide comprehensive care and achieve better health outcomes for the highest-need beneficiaries in Medi-Cal. The Lead Care Manager (LCM) plays an integral part in providing and coordinating care, including assessing the patient's condition and providing them the necessary resources, options, and coordination for a healthy care outcome.

What You’ll Do (just The Basics)

  • Supports and promotes the mission of the Fresno Mission.
  • Responsible for coordinating with the member’s service providers and support network
  • to ensure a seamless experience for the member and non-duplication of services.
  • Actively engage ECM-eligible members.
  • Oversee the provision of ECM services and implementation of the care plan.
  • Offer services where the member lives, seeks care, or finds most easily accessible and
  • within managed care plan guidelines.
  • Connect ECM members to other social services and supports they may need, including
  • transportation.
  • Advocate on behalf of ECM members with health care professionals.
  • Utilize motivational interviewing, trauma informed care, and harm reduction approaches.
  • Coordinate with hospital staff on ECM member discharge plans.
  • Accompany ECM members to office visits as needed and according to managed care plan
  • guidelines.
  • Monitor treatment adherence, including medication.
  • Provide health promotion and self-management training.
  • Carries a full caseload of 50 ECM members, unless instructed otherwise by senior
  • management within the scope of policy guidelines.
  • Identifies gaps in and barriers to service, documenting in the member’s care plan, and
  • communicating these to the multidisciplinary care team to develop and implement a
  • response.
  • Prepares for and participates in individual and group supervision meetings.
  • Behaves in an ethical, professional manner in support of Fresno Mission culture.
  • Displays positive and attentive attitude at site meetings, as well as department, program
  • and Fresno Mission staff meetings.
  • Works effectively with the program and site coordinators and the department head,
  • accepting direction and implementing policies and procedures reflective of this direction.
  • Displays sensitivity to the service population’s cultural and socioeconomics
  • characteristics.
  • Follows all policies, procedures and legal mandates including HIPAA, member
  • confidentiality, releases of information, protected health information (PHI) and member
  • files.
  • Reports to work on time and maintains reliable and regular attendance.
  • Performs other duties as assigned.

Education/Experience An AA or higher in the areas of Drug and Alcohol, Social Work, Human Services or related field.

Requirements A mature walk with Jesus Christ and able to articulate Biblical beliefs and their application within the context of the Fresno Mission. All employees of Fresno Mission are an integral part of the outreach ministry of a non‑denominational, evangelical ministry sharing the Gospel of Jesus Christ through transformational programs that provide food, shelter, clothing, education, job training and renewal of families to the poor and addicted. All employees are Christian missionaries and are required from time to time to participate in chapel services, outreach ministries, Bible studies and prayer times. Due to the nature of these types of ministries, it is an absolute necessity that each employee possesses and maintains a Christian testimony of their faith and experience in Jesus. Must be in agreement with the Statement of Faith of the Fresno Mission.
