I Am Boundless is seeking a Lead Care Coordinator for the OhioRISE program in Newark (hybrid). The role focuses on monitoring workflows, coaching staff, and ensuring timely referrals, care planning, and continuity of care across Outreach, Care Coordination, and Clinical Services.

• Requires leadership experience in behavioral health or care coordination.
• Experience coaching or training staff and reviewing documentation for quality.
• Knowledge of OhioRISE care coordination requirements is a plus.
• Strong organizational and problem-solving skills.
• Ability to manage multiple priorities and staff development initiatives.

Responsabilidades

• Serve as day-to-day resource for Outreach, Care Coordination, and Clinical Services staff on processes, timelines, and resources.
• Monitor workflows and address barriers; coordinate improvements with supervisors.
• Provide coaching, shadowing, and on-boarding support for new staff during the first 90 days.
• Lead portions of team meetings and trainings as assigned; ensure adherence to OhioRISE standards.
• Coordinate handoffs between Outreach, Virtual Care Coordination, and ongoing Care Coordination.
