Job summary

I Am Boundless in Newark, Ohio seeks a Lead Care Coordinator to bridge Outreach, Care Coordination, and Clinical Services. This role emphasizes monitoring workflows, coaching staff, and ensuring timely referrals and care planning across divisions.

The position involves guiding staff without formal supervisory authority, coordinating with supervisors, and supporting continuity of care while potentially handling a small temporary caseload. Hybrid work arrangement is available.

Qualifications

• High school diploma or GED required.
• Minimum three years of progressively responsible experience in children’s behavioral health, care coordination, case management, or related human services.
• Experience in leadership, mentoring, training, or project/workflow coordination.
• Experience with quality and documentation review and compliance monitoring.

Responsibilities

• Lead and support Outreach, Care Coordination, and Clinical Services staff; monitor workflows and timelines.
• Provide coaching, mentoring, and onboarding support for new staff (first 90 days).
• Lead portions of team meetings, huddles, and trainings; escalate clinical concerns to leadership.
• Coordinate workload and priorities across divisions with supervisors; ensure cross‑divisional alignment.
• Facilitate referrals, patient transitions, and communication among hospitals, providers, and families.
• Maintain documentation quality and compliance through chart reviews and audits.
