Job summary

I Am Boundless, Inc. is seeking a Care Coordinator Non-Licensed in Newark, OH to support youth and families in the OhioRISE plan. This role emphasizes assessment, linkage to services, and coordinated care through various communication channels.

The position is primarily community-based with potential work-from-home tasks and involves collaboration with Boundless staff, families, and service providers to meet health outcomes.

Qualifications

• Bachelor’s degree in psychology, social work, or related field OR ≥3 years in children's behavioral health.
• High School and GED required.
• Experience in family systems, community resources, case management, counseling, child protection, or child development.

Responsibilities

• Assess needs, linkage to services, and care coordination for youth and families in the OhioRISE plan.
• Interact with individuals, parents/guardians, Boundless staff and providers via in-person, phone, video, and electronic communication.
• Coordinate scheduling, track progress, exchange documentation, follow up on needs and appointments, and report outcomes.
