Job summary

I Am Boundless in Ohio is seeking a Care Coordinator to assess needs, link services, and coordinate care for youth and families under the OhioRISE plan. This is mainly community-based work with potential for some tasks from home and coordination with families, guardians, Boundless staff, and service providers.

Minimum qualifications include a Bachelor's degree in psychology, social work, or related field, or at least three years of relevant experience in children's behavioral health or public

Qualifications

• Bachelor’s degree in psychology, social work, or other related field OR at least three years of experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice or a related public sector human services or behavioral healthcare field, providing community-based services to children and youth, and their family or caregivers.
• High School and GED required
• Experience in one or more of the following areas of expertise: 1. Family systems; 2. Community systems and resources; 3. Case management; 4. Child and family counseling or therapy; 5. Child protection; 6. Child development
• Licensure/Certification: Valid Ohio Driver’s License with Ohio BMV - No more than 5 points on driving record; Valid car insurance; Current High Fidelity Wrap-Around and CANS Assessor training or ability to complete within 90 days of hire
• Training in cultural competency or ability to complete within 90 days of hire
• Ability to obtain and maintain First Aid, CPR - adult, child, and infant (training provided if needed)
• All candidates must complete background check, drug screen, and health screen as applicable
