Job summary

I Am Boundless in Ohio is seeking a Care Coordinator to support youth and families under the OhioRISE plan. You’ll assess needs, link services and coordinate care across providers in community settings, helping families navigate complex systems.

This role may include some work from home; you’ll engage with individuals, parents/guardians, Boundless staff and partners via in-person meetings, phone, video, and secure messaging to track progress and report outcomes.

Qualifications

• Bachelor’s degree in psychology, social work, or related field or at least three years of experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice or related public sector human services.
• High School diploma or GED required.
• Experience in family systems, community systems and resources, case management, child and family counseling or therapy, child protection, and child development.

Responsibilities

• Assess needs, service and resource linkage, and care coordination to support youth and families/caregivers under the OhioRISE plan.
• Care coordinators are primarily community-based and may perform some tasks from home.
• Serve as a primary point of contact and liaison among individuals, parents/guardians, Boundless staff, and other providers via face-to-face, telephone, video conferencing, and electronic communication.
• Schedule meetings, track progress, exchange documentation, follow up on needs and appointments, and report outcomes.
