Job summary

Integrated Services for Behavioral Health (ISBH) is seeking an OhioRISE Care Coordinator to plan and organize a team that supports youth at home, school, and in the community. You will lead family-centered care planning and ensure the voice of the youth and family is central in all decisions.

The role emphasizes collaboration with social service partners, documentation, HIPAA compliance, and travel within the service area to facilitate coordinated care.

Qualifications

• Experience providing services and/or support to children and families connected to behavioral health or related public sector human services.
• Two to three years in a coordinated supportive services or care coordination role with high efficiency and attention to detail.
• Ability to bill Medicaid without restrictions and to operate in an internet-based office environment.

Responsibilities

• Lead and organize child & family team meetings to support youth at home, school, and community.
• Develop care plans with families and partners to meet goals and reduce hospitalizations.
• Maintain documentation to access and sustain OhioRISE benefits and ensure HIPAA compliance.
• Travel within assigned service area and participate in trainings and supervision as required.
• Utilize EHR systems for accurate client documentation and service coordination.
