Providence Seaside Hospital seeks an LCSW for on-call, variable schedule. The licensed social worker independently conducts biopsychosocial/spiritual assessments, develops evidence-informed care plans, and coordinates with the IDT for both inpatient and outpatient populations on the Oregon coast.

Required: MSW from CSWE-accredited school, Oregon LCSW license, and 2+ years of social work experience (CSWA acceptable). Preferred: 1 year in acute care.

Qualificações

• Master's Degree in Social Work from a CSWE Accredited School, OR candidacy status by CSWE Council.
• Oregon Clinical Social Worker License on hire
• License in states where care is delivered
• 2+ years relevant Social Work experience (may include CSWA)

Responsabilidades

• Provide biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy.
• Collaborate with the IDT to develop and implement evidence-informed person-centered care plans across the care continuum.
• Serve both inpatient and outpatient populations across care continuum.
• Adhere to NASW codes of ethics and Standards of Practice and respect patient self-determination.
