Qualifications

• Graduate from an accredited nursing program.
• Current Connecticut RN license.
• BLS certification by American Heart Association.
• Two years of Medical-Surgical/Critical Care experience preferred.

Responsibilities

• Implements and monitors patient care plans; communicates patient condition.
• Acts as primary coordinator of multidisciplinary care for well-coordinated patient care.
• Follows physician and nursing orders; documents orders in the patient record.
• Assesses and coordinates discharge planning needs with the healthcare team.
• Delivers individualized patient care, addressing physical, psychosocial, cultural, spiritual, and age-related needs.
• Delegates care tasks to nursing staff to maximize patient outcomes.
