Job summary

COUNTY OF BROWN is seeking a dedicated Case Manager to support the CLTS Waiver Program for children with disabilities. You will develop individualized service plans, connect families to community resources, and advocate for family self-determination.

This role emphasizes collaboration with health and social services and requires monthly collateral contacts and documentation. The position may offer a hybrid schedule after an initial period, with a 37.5 hour work week and core hours 8:00am–4:30pm.

Qualifications

• Knowledge of case management principles and methods.
• Knowledge of family dynamics and human development.
• Knowledge of social work assessment principles and techniques.
• Knowledge of client rights, grievance and dispute resolution processes.
• Strong written and verbal communication skills.
• Ability to work independently and manage time.

Responsibilities

• Provide case management through CLTS Support and Service Coordination for children with disabilities.
• Create individualized service plans using available programs and resources.
• Connect families to community supports and promote family involvement.
• Maintain monthly collateral contacts and quarterly direct contact with families.
• Assist with eligibility, education, advocacy, and transitions to adulthood.
• Document accurately and maintain required documentation and files.
• Coordinate with healthcare providers, insurers, and social services to deliver services.
