Job summary

Lutheran Social Services of WI & Upper Michigan is seeking a full-time Care Coordinator to support children and families in the CLTS Waiver Program in Ozaukee County. You will help families access services and resources to enable children with developmental, physical, or emotional disabilities to thrive at home and in their communities.

This role features a hybrid work schedule with up to two remote days per week, Monday–Friday first shift hours, and a comprehensive benefits package including

Qualifications

• Bachelor's degree in Social Work or related Human Services field.
• At least one year of experience working with children and/or youth with disabilities, or an equivalent combination of education and experience.
• Valid driver's license and reliable transportation.

Responsibilities

• Serve as the primary point of contact for children and families enrolled in the CLTS program
• Complete assessments and identify service needs
• Develop and coordinate individualized, family-centered service plans
• Connect families with community resources and providers
• Facilitate team meetings and collaborate with service partners
• Maintain accurate documentation and client records
• Provide trauma-informed, person-centered support
