Job summary

Lutheran Social Services of WI & Upper Michigan is seeking a full-time Care Coordinator for the CLTS Waiver Program in Ozaukee County to help families access services and resources for children with disabilities. You will work to ensure safe, supportive home and community life and coordinate individualized service plans with a cross-disciplinary team.

Qualifications

• Bachelor's degree in Social Work or a related Human Services field.
• At least one year of experience working with children and/or youth with disabilities, or equivalent education/experience.
• Valid driver’s license and reliable transportation.

Responsibilities

• Serve as the primary contact for children and families enrolled in the CLTS program.
• Complete assessments and identify service needs.
• Develop and coordinate individualized, family-centered service plans.
• Connect families with community resources and providers.
• Facilitate team meetings with service partners.
• Maintain accurate documentation and client records.
• Provide trauma-informed, person-centered support.
