Job summary

Wood County Human Services Department is seeking a CLTS Support & Service Coordinator I/II to assess, plan and coordinate needed support for children with disabilities and their families. The role emphasizes service delivery, intake, and collaboration with schools and healthcare providers.

The position requires knowledge of CLTS/CCOP programs, billing and documentation, and travel to off-campus sites. A degree in human services and relevant experience are required, with compensation based on

Qualifications

• Associate degree in a human services-related field and at least two years of relevant experience required or four years of related experience in lieu of degree.
• Bachelor’s degree in health or human services and at least one year of professional experience required.

Responsibilities

• Assess, plan and coordinate needed support and services for children with disabilities (CCOP/CLTS).
• Provide direct supportive services to children and families, including information/referral and counseling.
• Conduct home visits, intakes, reviews and document relevant information in required systems.
• Complete required screens and paperwork (ISP, IEP reviews, funding requests, authorizations).
• Make home visits, attend school meetings and medical appointments as needed.
• Make contacts with clients, providers and professionals related to services.
