Job summary

Life Navigators in Milwaukee is hiring a CLTS Support and Service Coordinator to manage a caseload of 40–45 children, ages 0–18, under the CLTS Waiver Program. The role emphasizes advocacy, assessment, ISP development, and cross-system coordination to promote safe, independent living.

You will operate in a hybrid work environment with a focus on documentation, reporting, and family-centered service coordination. A background in social work and knowledge of public benefits are essential.

Qualifications

• Bachelor’s degree in Social Work, Public Health or related field required, Master’s preferred.
• One or more years of direct experience serving children, youth and individuals with disabilities; knowledge of public benefit programming.
• Excellent written and oral communication skills with clear, timely reporting.
• Knowledge of Milwaukee County service system and Wisconsin Children’s Waiver Programs.
• Proficient with Microsoft Office Suite and SUMAC Program Management data systems.

Responsibilities

• Provide comprehensive assessment, evaluation and monitoring of CLTS Care Management/Support.
• Ensure ISP outcomes, recertifications, required contacts, and incident reporting.
• Establish and monitor Individual Service Plans for children and families under CLTS.
• Maintain certification as a Children’s Long-Term Care Functional Screener.
• Coordinate with family members and service providers to promote safety and independence.
• Perform crisis intervention and coordinate with health/behavioral providers.
