Job summary

US Renal Care is seeking a Kidney Care Options Educator, Care Coordinator in the Dallas/Fort Worth area to promote and deliver the Kidney Care Options Program. The role supports multiple facilities, educates CKD/ESRD patients, and coordinates care transitions between dialysis modalities.

Under RN supervision, you will conduct group and individual classes, track program metrics, and maintain documentation. Strong customer service and communication skills are essential for success in this role.

Qualifications

  • Bachelor's degree or equivalent work experience in a healthcare setting.
  • Five years of experience in a healthcare organization working with patients and/or physicians.
  • Bi-lingual preferred. Excellent customer service and written/verbal communication skills.

Responsibilities

  • Promote and deliver the Kidney Care Options Program to nephrology offices, hospital case managers, and USRC staff.
  • Develop and sustain relationships with external and internal physicians/hospitals; provide follow-up to nephrologists.
  • Plan, schedule, and deliver group and individual Kidney Care Options classes for CKD stage 4 and 5 patients.
