Job summary

Fresenius Medical Care is hiring a Kidney Care Specialist to educate patients with CKD/ESRD and support care navigation across the kidney care continuum. The role blends field-based outreach with virtual patient support and requires collaboration with nephrologists and care teams to ensure timely, patient-centered education and referrals.

The specialist will use digital platforms and EMRs to document interactions, track engagement, and improve program outcomes.

Qualifications

• High School diploma or GED required; 3–5 years clinical background preferred.
• Relevant licensure or certification preferred, as applicable.
• Experience in kidney care or chronic disease management valued.

Responsibilities

• Provide virtual care navigation to CKD and ESRD patients regarding disease progression and treatment options.
• Conduct scheduled and on-demand virtual or in-person patient interactions to assess needs and barriers.
• Collaborate with nephrologists, care teams, and internal partners to support coordinated patient care and referrals.
