Job summary

Fresenius Kidney Care seeks a dedicated Kidney Care Specialist to educate, support, and navigate CKD/ESRD patients along the kidney care continuum. You will engage patients via virtual and in-person interactions, assess barriers, and coordinate with nephrology teams to optimize outcomes.

You will help educate patients on treatment options, including home modalities, and track engagement using CRM systems and EMRs. Travel and hybrid work are possible based on market needs.

Qualifications

• High School diploma or G.E.D. required; 3-5 years clinical background preferred.
• Relevant licensure or certification preferred, as applicable.

Responsibilities

• Provide virtualcare navigation to CKD and ESRD patients regarding disease progression, treatment options, and self-management strategies.
• Conduct scheduled and on-demand virtual or in-person patient interactions to assess needs, readiness, barriers, and preferences.
• Collaborate with nephrologists, care teams, and internal partners to support coordinated patient care and referrals.
• Build and maintain relationships with clinicians and care staff to support kidney care objectives.
