Job summary

U.S. Renal Care is seeking a Dietitian to join an interdisciplinary team delivering evidence-based nutrition care to patients with kidney disease.

Under supervision of an RDN Regional Manager, you will assess nutritional needs, tailor therapeutic diets, monitor labs, and educate patients and families. You will collaborate with medical directors, nurses, and social workers to support patient outcomes and participate in QAPI activities.

Responsibilities

• Patient nutrition: recommend therapeutic diets based on patient needs in consultation with the physician and review lab results with education on fluid/sodium/potassium and CKD-MBD management.
• Teamwork: collaborate with Medical Director, physicians, facility admin, social worker, nurse, and patient care tech to maintain relationships with hospitals, SNFs, agencies, vendors, and the community.
• Safety - Quality: support clinical/operational processes to improve patient outcomes and participate in monthly QAPI activities.
