Job summary

Avidity Integrated Care LLC is seeking a qualified professional to lead the development, implementation, and oversight of individualized support plans (ISPs) in alignment with NC Innovations Waiver requirements and agency policy. You will assess, plan, monitor progress, and collaborate with clients, families, and teams to ensure goals, strengths, and needs are reflected in each ISP.

Qualifications

• Degree in psychology, social work, counseling, or related human services field.
• Proficiency with ISP documentation systems (OnTarget preferred) and Microsoft Office.
• Strong person-centered planning, communication, and interpersonal skills.
• Must pass all applicable background checks and federal/state exclusion screening.

Responsibilities

• Develop, implement, monitor, and update ISPs in collaboration with persons served, families/guardians, and team members.
• Document assessments to inform ISP development.
• Coordinate ISP team meetings and document outcomes and follow-up.
• Ensure ISPs reflect individual goals, strengths, needs, and preferences.
