Avidity Integrated Care LLC is seeking a qualified professional to lead the development, implementation, and oversight of individualized support plans (ISPs) in alignment with NC Innovations Waiver requirements and agency policy. You will assess, plan, monitor progress, and collaborate with clients, families, and teams to ensure goals, strengths, and needs are reflected in each ISP.

Position Summary

Serves as the primary professional responsible for development, implementation, and oversight of individualized support plans (ISPs) in compliance with NC Innovations Waiver requirements and agency policy.

Essential Duties and Responsibilities

• Conduct and document comprehensive assessments of persons served to inform ISP development.
• Develop, implement, monitor, and update person-centered ISPs in collaboration with persons served, families/guardians, and team members.
• Ensure ISPs reflect individual goals, strengths, needs, and preferences and are reviewed at required intervals.
• Provide or coordinate training to direct support professionals (DSPs)/paraprofessionals (PP) on ISP implementation.
• Monitor progress toward ISP goals and maintain required documentation in OnTarget or the designated electronic system.
• Facilitate and attend ISP team meetings and document outcomes and required follow-up.
• Ensure compliance with NC Innovations Waiver billing, authorization, and documentation requirements.
• Identify and report any suspected abuse, neglect, or exploitation in accordance with agency policies.
• Participate in performance improvement activities and provide input on service quality and outcomes.
• Maintain required QP credentials and stay current in field-relevant knowledge.

Minimum Qualifications

• Meets NC DMA Qualified Professional criteria: relevant degree in psychology, social work, counseling, or a related human services field and one year of experience with the IDD population, or as otherwise specified by Medicaid/Alliance requirements.
• Proficiency with ISP documentation systems (OnTarget preferred) and Microsoft Office.
• Strong person-centered planning, communication, and interpersonal skills.
• Must pass all applicable background checks and federal/state exclusion screening.
