Job summary

Qualifications

  • Valid driver's license and ability to travel as required.

Responsibilities

Job description

The Program Specialist coordinates and monitors activities related to the development, implementation, and documentation of Individualized Service Plans (ISPs) for individuals receiving services. This role collaborates with program leadership and staff to support person-centered planning, ensure ISP compliance, maintain documentation standards, and promote quality service delivery.

The Program Specialist serves as a resource on ISP processes, participates in quality assurance activities, and contributes to Positive Behavioral Supports (PBS) initiatives across the organization.

Major Responsibilities
ISP Coordination & Monitoring
  • Monitor ISP activities within the Home and Community Services Information System (HCSIS).
  • Review and respond to alerts related to ISP deadlines and requirements.
  • Track documentation timelines and submission requirements.
  • Communicate HCSIS-related concerns and compliance issues to leadership.
ISP Planning & Facilitation
  • Schedule and facilitate ISP planning meetings with program staff and stakeholders.
  • Assist teams in identifying meaningful, measurable goals and objectives aligned with individual needs and preferences.
  • Support the development of data collection methods used to monitor progress toward ISP outcomes.
  • Review progress toward established objectives and assist in updating plans as needed.
  • Track and coordinate ISP documentation throughout the planning and approval process.
Documentation & Quality Review
  • Conduct semi-annual reviews of ISP-related documentation and records.
  • Provide feedback and guidance to managers regarding compliance and documentation standards.
  • Prepare reports summarizing ISP documentation status, trends, and areas for improvement.
  • Support organizational quality assurance efforts related to ISP implementation.
Satisfaction Surveys
  • Conduct satisfaction surveys with individuals receiving services prior to ISP meetings.
  • Coordinate survey completion for individuals who require communication supports.
  • Collect and manage annual family and guardian satisfaction surveys.
  • Conduct follow-up discussions regarding concerns identified through survey responses.
Training & Development
  • Assist in training managers and staff on ISP requirements, best practices, and compliance expectations.
  • Support the development and delivery of ISP-related training materials.
  • Participate in the design and implementation of Positive Behavioral Supports (PBS) training initiatives.
  • Serve as a member of the Positive Behavioral Supports Steering Committee.
Additional Duties

Qualifications
Required Education & Experience
  • Bachelor's degree required.
  • Minimum of two (2) years of experience working with individuals with intellectual and developmental disabilities (IDD).
  • Knowledge of DDS ISP systems, processes, and regulatory requirements.
  • Proficiency in Microsoft Office Suite and related computer applications.
  • Valid driver's license with the ability to travel as required.

Working Conditions & Physical Requirements
  • Primarily office-based work with occasional travel to program sites and meetings.
  • Flexible scheduling may be required to accommodate individuals served and family participation in interviews or planning meetings.
  • May be exposed to varying environmental conditions when visiting community or residential settings.
  • Exerts up to 10 pounds of force occasionally.
  • Primarily involves sitting for extended periods while performing administrative and computer-based tasks.
