Abound Health is seeking a Qualified Professional to support and advocate for individuals receiving Innovations Waiver or State Funded Services. You will help people live meaningful lives and ensure the highest level of care and support.

Responsibilities include developing short-term goals, supervising staff, and leading ISP meetings. Candidates should have relevant education and IDD experience; hybrid work is noted, with comprehensive benefits and a 25% bonus potential.

Qualifications

• Bachelor’s degree in a related human service field with 2 years of full-time IDD experience, OR a related field with 4 years.
• Master’s degree in a related human service field with 1 year of full-time IDD experience.

Responsabilités

• Develop individualized short-term goals and interventions.
• Conduct clinical and employee supervisory visits, ensuring high standards of care and compliance.
• Provide ongoing development plans and annual performance evaluations for direct care staff.
• Participate and advocate for recipients in ISP meetings.
• Monitor billing, maintain documentation, and resolve grievances.
• Represent Abound Health at community events and outreach.
