Job summary

COMHAR is seeking a skilled Program Specialist to coordinate and implement Individual Support Plans (ISPs) for individuals with IDD and behavioral health needs. You will work closely with participants, families, and staff to ensure services meet individualized needs and promote independence and community integration.

The role emphasizes coordinating ISP assessments, collaborating with DSPs, and ensuring regulatory compliance while maintaining consumer dignity and safety in a nonprofit,

Qualifications

• Associates or 60 college credits with at least 3 years related experience in ID required (must).
• Bachelor’s degree in Human Services, Social Work, Psychology, or related field preferred.
• Current PA Driver's License; driving record satisfactory; able to operate program vehicles.

Responsibilities

• Coordinate and complete initial, annual, and revision assessments for assigned caseload.
• Disseminate assessments to ISP team members at least 30 days prior to ISP meetings.
• Provide training to DSPs on ISP-related plans and medication protocols.
