You will coordinate person-centered planning, monitor ISP documentation, and support staff trainings while aligning with internal and external standards. This role offers growth opportunities within a mission-driven organization dedicated to meaningful community integration.

Qualifications

• Bachelor's degree in a related field.
• Three years of work experience in developmental disabilities.
• Root Cause Analysis training and DBHDS-related credentials.
• Pass Virginia DBHDS fingerprinting & background checks and ODD assessment.

Responsibilities

• Coordinate and participate in person-centered ISP planning and outcomes.
• Coordinate documentation of SIS scores, Waiver ISPs, and WaMS approvals.
• Coordinate referrals, admissions, transfers, and discharges and outreach.
• Review progress and provide feedback to ensure quality of services.
• Develop staff trainings on documentation standards.
