The role emphasizes self-direction, data-driven decision making, and effective communication with participants, DHS directives, and external vendors, with occasional travel to meet program needs.

Qualifications

• Minimum 5 years health care with care coordination experience.
• At least 2 years serving IRIS target groups (adults with disabilities or older adults) or equivalent.
• Valid driver's license with reliable transportation for travel.
• Strong leadership, performance management and data analytics skills.
• Knowledge of long-term care programs and self-determination principles.

Responsibilities

• Leads and supervises a regionally-based team of IRIS consultants to ensure high-quality, person-centered supports.
• Reviews data and reports to manage IRIS services and identify proactive solutions.
• Conducts monthly reviews of consultant records, ensuring timelines and plan accuracy.
• Authorizes participant plans, budget amendments, and liaises with vendors as needed.
• Communicates program changes and budget impacts clearly to the IRIS team and participants.
• Manages confidentiality under HIPAA and maintains professional relationships across a matrixed organization.
• Engages external stakeholders and conducts outreach to promote service excellence.
• Attends regular leadership meetings and annual home visits with IRIS consultants.
• Occasional local travel may be required based on state/contractual requirements.
