Your schedule will be divided between both areas of responsibility while collaborating closely with the interdisciplinary care team.ResponsibilitiesMDS CoordinationComplete and coordinate MDS assessments in accordance with state and federal regulationsEnsure accurate coding to support reimbursement and quality measuresDevelop, update, and monitor individualized resident care plansCollaborate with the interdisciplinary team to promote exceptional resident outcomesMaintain accurate and timely clinical documentationInfection PreventionLead the facility's Infection Prevention and Control ProgramConduct infection surveillance, tracking, investigations, and required reportingAudit infection control practices throughout the facilityEducate staff on infection prevention standards and best practicesMonitor compliance during all shifts, including periodic off-shift observationsEnsure compliance with CDC, CMS, and Illinois Department of Public Health (IDPH) infection prevention guidelinesQualificationsCurrent Illinois Registered Nurse (RN) license requiredPrevious MDS experience requiredInfection Prevention experience preferred, but not requiredWillingness to complete the CDC Infection Preventionist Training if not already certifiedStrong assessment, organizational, communication, and leadership skillsAbility to work independently while collaborating effectively with an interdisciplinary teamParticipation in the facility's management on-call rotation is requiredJoin Our Leadership TeamAt Cass County Senior Living & Rehabilitation, exceptional resident care begins with exceptional clinical leadership.
