Job summary

Stonebridge Nursing & Rehabilitation is seeking an organized RN or LPN to join our leadership team as the MDS Coordinator in Benton, IL. This role oversees MDS assessments, documentation, and coordination with the interdisciplinary team to support care quality and regulatory compliance.

The position offers a full-time nursing leadership opportunity with competitive RN/LPN wages ($33-$35/hr for RN, $29-$31/hr for LPN) plus on-call pay and a benefits package including BCBS, 401(k), and paid

Qualifications

• Current Illinois RN or LPN license in good standing.
• Strong clinical assessment and documentation skills.
• Excellent organizational and time-management abilities.
• Strong communication and critical-thinking skills.
• Ability to collaborate effectively with the interdisciplinary team.
• Long-term care or skilled nursing experience preferred.
• Previous MDS experience preferred.

Responsibilities

• Coordinate and oversee the MDS assessment process.
• Complete accurate and timely MDS assessments and submissions.
• Coordinate resident care planning with the interdisciplinary team.
• Monitor clinical documentation for accuracy and completeness.
• Support reimbursement accuracy and regulatory compliance.
• Monitor quality measures and resident outcomes.
• Assist with survey readiness and quality improvement initiatives.
• Communicate with nursing staff regarding documentation and resident care needs.
• Work closely with facility and nursing leadership.
