Job summary

Carrier Mills Nursing & Rehabilitation Center LLC in Carrier Mills, IL is seeking an experienced MDS Nurse Manager (RN or LPN) to lead the MDS process and ensure regulatory compliance.

You will coordinate assessments, guide care planning with an interdisciplinary team, monitor documentation and reimbursement, and work with nursing leadership to improve resident outcomes and quality measures.

Qualifications

• Requires Illinois RN or LPN license and MDS/care planning experience.
• Experience with Medicare/Medicaid reimbursement is preferred.

Schedule

• Full-Time
• Monday–Friday
• 8:00 AM – 4:30 PM
• Participation in the management on-call rotation is required

Compensation

• Registered Nurse (RN): $33.00–$35.00/hour
• Licensed Practical Nurse (LPN): $29.00–$31.00/hour
• $10,000 Sign-On Bonus
• Sign-on bonus paid over one year
• Annual performance evaluation with the potential for a pay increase

What You'll Do

As our MDS Nurse Manager, you'll oversee and coordinate the MDS process while working closely with nursing leadership and the interdisciplinary team to promote accurate documentation, regulatory compliance, reimbursement accuracy, and excellent resident outcomes.

What We're Looking For

• Ability to collaborate effectively with an interdisciplinary team
