Job summary

Colonial-Vista-Post-Acute in Wenatchee, WA is seeking an experienced MDS Coordinator RN to oversee the MDS process, ensure regulatory compliance, and collaborate with the interdisciplinary team to deliver quality resident care.

This full-time role offers a $5,000 sign-on bonus and a salary range of $115,000 to $125,000 per year DOE, plus benefits including health insurance, 401(k), PTO, and holidays. A strong emphasis on accuracy, organization, and leadership will be essential.

Qualifications

• Active RN license in the state of practice.
• MDS experience preferred.
• Knowledge of Medicare/Medicaid regulations for skilled nursing.
• Experience with PointClickCare (PCC) preferred.
• Strong organizational, leadership, and communication skills.

Responsibilities

• Coordinate and complete MDS assessments accurately and on time.
• Develop, update, and maintain resident care plans.
• Manage Medicare and Medicaid assessment schedules and documentation.
• Ensure compliance with federal, state, and facility regulations.
• Collaborate with physicians, nursing staff, therapy, and other interdisciplinary team members.
• Maintain accurate resident documentation and medical records.
• Participate in quality assurance initiatives and regulatory surveys.
