Job summary

Legacy Healthcare LLC in Illinois seeks an MDS Coordinator RN to support our skilled nursing and rehabilitation facility. You will develop, monitor, and modify each resident’s care plan, perform assessments, and assist in discharge, ensuring high-quality care tailored to individual needs.

Applicants should hold an IL RN license, be a nursing program graduate, CPR certified, with prior MDS coordination and SNF RN/CNA experience.

Qualifications

• Unencumbered State of Illinois RN license.
• Graduate of an accredited nursing program.
• CPR certification.
• Prior experience as an MDS Coordinator.
• One (1) year RN experience in a skilled nursing setting.
• One (1) year CN A experience.

Responsibilities

• Develop, monitor, audit and modify each resident’s care plan to meet individual needs and goals.
• Perform resident assessments and assist in the discharge process.
• Ensure residents receive a high standard of care through attention to detail.

An MDS Coordinator is instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

• As a minimum, an unencumbered State of Illinois R.N. License;
• A graduate of an accredited nursing program
• C.P.R. Certification.
• Prior experience as a MDS Coordinator;
• One (1) year of experience as an R.N. in a skilled nursing setting
• One (1) year of experience as a C.N.A.
