Job summary

PATH in San Diego, CA is seeking a Case Manager for the Veteran GPD Program to help clients exit homelessness through housing-focused services and a “whatever it takes” approach. The role supports individuals through street outreach, interim housing, and permanent housing with coordinated access to healthcare, social services, and support systems.

The Case Manager conducts intake, develops ISP plans, provides referrals, completes VE-SPDAT/assessments, and evaluates progress with 90-day reviews

Qualifications

• Two years of experience in healthcare, case management, homeless services or related field
• All levels of education and experience welcome
• CPR/First Aid training or ability to obtain it
• Eligibility to work in the U.S. without sponsorship

Responsibilities

• Conduct intake and develop an Individualized Service Plan (ISP) addressing housing goals and income.
• Provide referrals, linkages, and support to resources to achieve ISP goals.
• Complete program assessments (e.g., VI-SPDAT).
• Evaluate progress with 90-day reviews and adjust plans as needed.
