Job summary

Adjoin in San Diego is seeking a Housing Services Specialist to collaborate with Veterans and the team to identify and secure affordable housing. You will deliver direct services to Veterans and their families who are homeless or at imminent risk.

Qualifications

• Bachelor’s degree or two years of equivalent experience in working with veterans or homelessness.
• Valid driver’s license and insured vehicle.
• Must pass pre-employment drug screen and background check.
• Ability to travel within and throughout the counties served.
• CPR/First-Aid certification or completion within 45 days of hire.
• Must successfully complete Adjoin training programs.

Responsibilities

• Complies with all safety standards and practices. Transport Veterans to appointments, housing searches, or other necessary locations, ensuring safety and timeliness during travel.
• Maintain case notes in an accurate and detailed manner. Maintain positive and professional communications and relationships with Veterans/individuals, household members, and collateral contacts.
• Provide direct services to a wide range of Veterans and their families that are homeless or at imminent risk.
• Collaborate with Veterans and other team members to identify and secure affordable housing.
• Manage all paperwork and meet all coinciding deadlines.
• Completes all training within required deadlines.
• Develop Housing Stability Plans (HSP) and action steps to achieve and maintain permanent housing for Veterans.
• Provide ongoing referrals and resources to Veterans throughout their time in the program and document on Referral Log and case notes.
