Job summary

PATH is seeking a Case Manager for the Zephyr Veteran PSH site in San Diego, CA. You will support clients in breaking the cycle of homelessness through a flexible, client-centered approach.

You will conduct intake, develop Individualized Service Plans, coordinate referrals, and monitor progress to ensure housing goals are met while maintaining ethical standards and strong collaboration with partners and participants.

Responsibilities

• Conduct intake and needs assessment and develop ISP addressing housing goals and resources.
• Provide referrals and linkages to services supporting ISP goals.
• Complete program assessments (e.g., VI-SPDAT) and monitor progress.
• Evaluate progress and conduct periodic 90-day reviews with participants.
