Job summary

PATH (People Assisting the Homeless) is seeking a Case Manager for the Interim Housing team at the Luxury Inn location in Long Beach, CA. You will assist clients through the continuum of housing options, coordinating access to healthcare, social services, and supportive services to help secure and maintain housing.

The role emphasizes individualized planning, collaborative case management, and trauma-informed care to empower clients facing homelessness. �

Responsibilities

• Conducts intake and needs assessment and develops ISP aligned with housing goals.
• Provides referrals and support to resources to achieve ISP goals.
• Completes program assessments (e.g., VI-SPDAT).
• Evaluates progress with participants and conducts reviews.
• Maintains confidential participant files and HIPAA compliance.
• Develops therapeutic relationships and provides crisis intervention as needed.
