Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington in Sugar Land, TX is seeking a Case Manager to coordinate interdisciplinary care, review admissions, and plan safe discharges for our patient population.

You will work with nurses, therapists, and social workers to optimize resources, improve outcomes, and strengthen payor relationships. A current BLS cert and state licensure are preferred, with 3–5 years inpatient experience desirable.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Discipline options may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
• Current BLS certification required.

Responsibilities

• Coordinate interdisciplinary care for designated patient populations.
• Review inpatient admission records to ensure appropriate level of care.
• Assess discharge needs and coordinate discharge plan.
• Collaborate with interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
