Job summary

Licensure in the resident state and current BLS certification are required, with 3–5 years inpatient experience preferred in acute, IRF, or LTACH settings.

Qualifications

• Licensure in the state where the hospital resides.
• Professional disciplines may include OT, PT, RN, RT, SLP, or social worker.
• Current BLS certification required.
• 3–5 years inpatient experience, preferably in an acute, IRF, or LTACH setting.

Responsibilities

• Coordinate care for designated patient population.
• Review inpatient admission records for appropriate level of care.
• Assess discharge needs and coordinate discharge planning.
• Collaborate with interdisciplinary team to improve financial and quality outcomes.
• Maintain and enhance payor relationships.
