Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington in Corpus Christi, TX seeks a Case Manager to coordinate interdisciplinary care for a defined patient population, review admissions, and determine appropriate levels of care.

You will collaborate with the care team to ensure discharge planning meets financial and quality goals and maintain payer relationships; state licensure and BLS are preferred.

Qualifications

• State licensure in the hospital's state preferred.
• Three to five years inpatient experience preferred (IRF/LTACH/acute).
• BLS certification required.

Responsibilities

• Coordinate interdisciplinary care for designated patient populations.
• Review inpatient admission records for proper resource utilization and level of care.
• Identify discharge needs and coordinate discharge planning.
• Collaborate with the interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
