Job summary

PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Case Manager to coordinate interdisciplinary care for a patient population and ensure appropriate admission and discharge planning. This role collaborates with the care team to achieve desired financial and quality outcomes and to strengthen payor relationships.

Education includes state licensure (preferred) and required BLS; experience of 3–5 years in inpatient settings such as acute, IRF, or LTACH is expected.

Qualifications

• Education and Training: Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or social worker.
• Current BLS certification required.

Responsibilities

• Interdisciplinary coordination of care for a designated patient population.
• Review inpatient admission records to ensure proper utilization of hospital resources.
• Assess discharge needs and coordinate discharge plans.
• Maintain and enhance payor relationships.
