The role requires licensure in the state and a current BLS certification, with 3–5 years of inpatient experience preferred in acute or related settings.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or social worker.
• Current BLS certification required.

Responsibilities

• The Case Manager coordinates care across disciplines for a designated patient population.
• Reviews inpatient admission records to ensure proper utilization of hospital resources and admission decisions.
• Assess discharge needs and coordinate the discharge plan.
• Collaborates with the interdisciplinary team to meet financial and quality outcomes.
• Maintains and enhances payer relationships.
