PAM Health Specialty and Rehabilitation Hospital of Covington is seeking a Case Manager to coordinate interdisciplinary care for a designated patient population within an inpatient setting. You will review admissions, ensure appropriate care levels, assess discharge needs, and partner with the care team to improve outcomes.

The role requires state licensure in the hospital's location (preferred) and current BLS certification, with 3–5 years of inpatient experience in acute/IRF/LTACH settings.

Competenze

• Licensed in the state where the hospital resides is preferred; professional disciplines may include OT, PT, RN, RT, SLP, or social worker.
• Current BLS certification required.
• Three to five years of inpatient experience, preferably in an acute, IRF, or LTACH setting.
